First Visit |
We are always excited about meeting new patients during their first visit. You do not need a referral to come to see us. Your one hour initial appointment will consist of two low dose direct digital x-rays, a thorough examination, and a discussion of your treatment options and cost. The initial examination is complimentary.
We know your time is valuable, so if you want to expedite your start, let us know and we can also schedule the remaining diagnostic records consisting of impressions for study models, bite registration and digital photos at the first visit. These are necessary to develop an appropriate treatment plan in detail. We feel it is important to analyze each patient’s specific needs, because in-depth planning helps us to achieve our goals of outstanding results and high patient satisfaction.
During the initial examination Dr. Neely will address the following questions that cover the basics of orthodontic treatment. We encourage you to ask questions as well.
- Is this a condition that orthodontics can help?
- Should treatment start now, or would it be better to wait?
- What treatment procedures or appliances are recommended to correct the problem?
- Do any teeth have to be removed?
- How long will treatment take? Will there be more than one phase?
- Is referral to anther dental specialist, otolaryngologist (ENT), or allergist recommended?
- How much will it cost?
- Will my insurance cover part of it? Please bring your insurance card with you.
- What are my payment options?
After the examination, our Treatment Coordinator will review what was discussed. We want our new patients to leave our office with a clear understanding of their specific needs, what the recommended treatment will consist of, and how long it is expected to take.
New Patient Registration
Please complete the Patient Registration/Medical/Dental Health History Form on the screen and submit it online or print it and fax, or send it to us before your visit. Dr. Neely will review the information as soon as we receive it. Because of the overlapping nature of the medical, dental, and growth related factors involved in planning orthodontic treatment, the information you provide will allow Dr Neely to be prepared for your consultation. Dr. Neely may want to consult with your dentist or physician for additional information or to coordinate treatment.

- TMD Problems (For patients whose jaw joint clicks or hurts, or who grind their teeth in their sleep.)
